Qualifications

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and function.
• Supervise Physical Therapy Assistants in resident care.
• Communicate with physicians, nursing staff, families about progress and discharge planning.
• Document timely, accurate, and compliant records per policies and payer requirements.
• Educate residents and families on home exercises, safety, and fall prevention.
• Maintain infection control and equipment safety in the treatment area.
• Participate in care conferences and quality improvement activities.
