Job summary

Primrose Post Acute is seeking a Staff Physical Therapist to evaluate residents, develop individualized treatment plans, and improve mobility and independence. The role includes supervising PT assistants, coordinating with a multidisciplinary team, and providing in‑service training for staff.

The ideal candidate will hold a California Physical Therapist license (or be eligible) with a bachelor's degree in physical therapy and, preferably, a Master's or Doctorate.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Licensed in California as a Physical Therapist.
• Masters or Doctorate in Physical Therapy preferred.
• CPR certification may be required by licensure.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and supervise direct resident care.
• Communicate progress and goals with physicians, staff, residents and families.
• Complete accurate documentation in accordance with regulations and payer requirements.
