Job summary

You will collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other professionals in a supportive interdisciplinary team as the facility transitions toward Willow Springs Post Acute, offering a stable full-time role

Qualifications

• Current Physical Therapist license in good standing.
• Strong clinical reasoning and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to thrive in a collaborative healthcare environment.
• Skilled nursing or post-acute experience preferred but not required.

Responsibilities

• Evaluate patients and establish individualized therapy plans.
• Develop functional goals that support recovery and independence.
• Implement therapeutic exercise programs and mobility training.
• Monitor patient progress and modify treatment plans as appropriate.
• Educate patients and families regarding treatment goals and home exercise programs.
• Collaborate with interdisciplinary team members to support successful patient outcomes.
• Complete timely and accurate clinical documentation.
• Participate in discharge planning and care conferences.
