Job summary

Corewell Health is seeking a Pharmacist for the Mehney Discharge Lounge Clinic in Grand Rapids to support patients transitioning from inpatient to home. The role emphasizes medication reviews, reconciliation, education, and collaboration with physicians, nurses, and care teams to reduce readmissions.

The ideal candidate has a PharmD, a PGY1/PGY2 residency preferred, and licensure in Michigan. The position is on-site, 24 hours per week, with Tue/Fri and one midweek day scheduled.

Qualifications

• Bachelor's Degree in Pharmacy or Doctor of Pharmacy from an accredited College of Pharmacy.
• 2 years of relevant experience pharmacy experience or an ASHP Pharmacy Practice Residency required.
• 2 years of relevant hospital pharmacy experience or an ASHP Pharmacy Practice Residency preferred.
• ASHP accredited PGY2 residency in area of specialty preferred.
• MI licensure requirements within 90 days of hire; BLS/PALS/ACLS certifications may be required as specified.

Responsibilities

• Coordinate educational programs to enhance clinical drug knowledge of staff pharmacists and other caregivers.
• Provide pharmaceutical care, drug information, and education to hospital staff, physicians, patients and families.
• Lead drug utilization review efforts and process-improvement activities to optimize therapy and costs.
• Assist with high-risk medications initiation, monitoring, and pharmacokinetic interpretation.
• Precept pharmacy residents and students as assigned.
