Qualifications

• Bachelor's Degree or PharmD from an accredited College of Pharmacy.
• ASHP-accredited residency in Pharmacy Practice preferred.
• MI Pharmacist license required; State MI Pharmacy licensure.
• Certified in Basic Life Support (BLS) per AHA/ARC.

Responsibilities

• Provide daily patient-focused clinical pharmacy services for inpatients and outpatients, including medication dosing and chart review.
• Enter orders and perform staff pharmacist duties; participate in committees and education programs.
• Educate patients and community; provide clinical training to PharmD students as assigned.
• Document interventions and adverse drug reactions accurately and timely.

While this pharmacy provides traditional outpatient pharmacy services for the community, its main purpose is to operate a Discharge Medication Program to collaboratively work across professional disciplines to ensure patients discharging from the hospital leave with their new medications in hand. Corewell Health Grand Rapids Hospitals Retail Pharmacy is a closed-door, outpatient pharmacy located at Butterworth Hospital in Grand Rapids, Michigan. This pharmacy operates under a “Specialty-Plus” Model, dispensing both Specialty and Mon-Specialty Medications pertinent to specific disease states that often require a higher level of care. This frequently involves a multidisciplinary, collaborative approach to ensure proper medication coordination and administration. Patient populations currently served include Hemophilia and Cystic Fibrosis.

Scope of Work

• Provide daily patient focused clinical pharmacy services to inpatients and outpatients of all age groups of Lakeland Regional Health System to include, but not limited to, medication dosing, chart review, patient education, floor rounds and physician intervention. Participate on committees and give special reports. Rotate through the distributive pharmacy as scheduled.
• Directly perform order entry in the pharmacy; Provide all of the functions as a staff pharmacist; Perform other duties as assigned.
• Provide patient and community education; Participate in the staff development education program; Provide clinical training to Pharm. D. students as assigned.
• Perform accurate and timely input of interventions and ADR’s.
• Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service. Adhere to the organization’s published values

Qualifications

• Required Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
• 2 years of relevant experience
• American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice Preferred
• LIC-Clinical Pharmacist - STATE_MI State of Michigan
• LIC-Pharmacist Controlled Substance - STATE_MI State of Michigan
• CRT-Basic Life Support (BLS) - AHA American Heart Association
• CRT-Basic Life Support (BLS) - ARC American Red Cross
• Ideal candidate will be a Pharmacist possessing PharmD Degree and/or PGY1 Residency and/or PGY2 in Retail / Community / Ambulatory Pharmacy
