Job summary

Rotech Healthcare is seeking a Hospital Liaison to coordinate patient discharges and ensure timely delivery and setup of home respiratory and DME equipment. You will educate patients and caregivers, maintain documentation, and support emergency equipment needs while traveling to patient homes in the field.

The role involves coordinating with hospital teams, aligning with Case Management and Continuing Care, and contributing to quality improvement efforts.

Qualifications

• High school diploma or GED required.
• Healthcare experience preferred.
• Valid driver’s license required in state of residence.

Responsibilities

• Coordinate patient discharges with hospital case managers and staff.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on proper equipment use and maintenance.
• Maintain accurate documentation including referral forms, insurance verification, and delivery tickets.
• Support emergency equipment needs and troubleshoot issues as they arise.
• Participate in quality improvement initiatives to meet standards.
• Travel to patient homes using a company vehicle.
• Serve as a trusted discharge planning resource for hospital staff.
