Rotech Healthcare Inc. is seeking a Hospital Liaison – Respiratory & DME Services to coordinate patient discharges and ensure seamless transition to home care.

You will work with Case Management and Continuing Care teams to arrange timely equipment delivery and educate patients and caregivers on proper use. The role involves travel to patient homes using a company vehicle, collaboration with hospital staff, and contributing to quality improvement efforts while supporting discharge planning

Formación

• Background check, drug screen, and credentialing as required.
• Valid driver’s license with a clean driving record.
• Experience with medical equipment or healthcare setting preferred.

Responsabilidades

• Coordinate patient discharges with hospital case managers and staff.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on equipment use and maintenance.
• Document referral forms, insurance verification, delivery tickets and logs.
• Support emergency equipment needs and troubleshoot issues as they arise.
• Participate in quality improvement initiatives to meet standards.
