Job summary

Rotech Healthcare Inc. is seeking a Hospital Liaison to connect hospital staff with our home care services. You will coordinate discharge planning, arrange timely delivery and setup of respiratory equipment, and educate patients and caregivers on proper use.

This role requires clear English communication, strong organizational skills, and the ability to work independently, with travel to patient homes and collaboration with Case Management and Continuing Care teams.

Qualifications

• High school diploma or GED required.
• Experience in healthcare coordination preferred.
• Background check and drug screen as applicable.
• Valid driver’s license with clean record when required.

Responsibilities

• Coordinate patient discharges with hospital staff and case managers.
• Ensure timely delivery and setup of respiratory equipment at discharge.
• Educate patients and caregivers on equipment use and maintenance.
• Maintain documents: referrals, insurance verification, tickets, logs.
• Support emergency equipment needs and troubleshoot issues.
• Participate in quality improvement initiatives per standards.
• Maintain relationships with hospital personnel and discharge planning.
• Provide in-service training and CEU programs as needed.
• Represent the company at trade shows and events.
• Travel to patient homes for equipment setup using company vehicle.
