Job summary

Rotech Healthcare is seeking a Hospital Liaison to coordinate discharges and ensure smooth transitions from hospital to home care. You will work with Case Management and Continuing Care teams to arrange timely delivery of Respiratory Therapy and DME, educate patients, and document all activities.

The role requires coordinating with hospital staff, traveling to patient homes for equipment setup, and meeting performance standards. A valid driver’s license and healthcare knowledge are preferred.

• Any education requirements mandated by applicable law, regulation, licensing, or customer contract.

Responsibilities

• Coordinate patient discharges with hospital case managers, social workers, nurses, and physicians.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on proper equipment use and maintenance.
• Maintain accurate documentation including referral forms, insurance verification, delivery tickets, and activity logs.
• Support emergency equipment needs and troubleshoot issues as they arise.
• Participate in quality improvement initiatives to meet company standards.
