Job summary

Rotech Healthcare is seeking a Hospital Liaison to connect hospital teams with our home care services. You will coordinate discharge planning, arrange respiratory and DME equipment delivery, and educate patients and caregivers on safe use.

The role includes travel to patient homes, collaboration with Case Management and Continuing Care, and a bonus opportunity based on performance. Hours align with hospital discharge needs and on-site visits in Florida.

Qualifications

• High school diploma or GED required or equivalent experience.
• Education requirements may be mandated by law, regulation or customer contract.
• Combination of education, training, and relevant work experience may qualify candidates.

Responsibilities

• Coordinate patient discharges with hospital staff and case management.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on equipment use and maintenance.
• Maintain accurate documentation including referrals, insurance verification, and delivery tickets.
• Travel to patient homes using a company vehicle for equipment setup and follow-up.
