Job summary

Rotech Healthcare Inc. is seeking a Hospital Liaison to support coordination between hospital staff and our home care services. You will work with Case Management to ensure smooth transitions for patients needing Respiratory Therapy and DME at home, coordinating discharge planning and equipment delivery.

You will educate patients and caregivers, maintain documentation, and travel to patient homes for setup using a company vehicle, contributing to safe, timely home transitions.

Qualifications

• High school diploma or GED required.
• Experience with medical equipment or in a healthcare setting preferred.
• Strong communication and organizational skills; ability to handle confidential information.

Responsibilities

• Coordinate patient discharges with hospital staff and case management.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on proper equipment use and maintenance.
• Maintain accurate documentation including referrals, insurance verification, and delivery logs.
• Travel to patient homes for equipment setup using a company vehicle.
