Job summary

Rotech Healthcare Inc. seeks a Hospital Liaison to coordinate transitions for patients needing Respiratory Therapy and DME at home. You will partner with Case Management and Continuing Care to ensure smooth discharges and timely equipment delivery.

Responsibilities include education for patients/caregivers, documentation, and support for equipment troubleshooting. Schedule: Tue-Fri 8-5 and Sat 9-6; compensation includes a bonus opportunity.

Qualifications

• Background check and drug screen may be required.
• Valid driver's license with a clean driving record.
• Compliance with credentialing processes when applicable.
• Education requirements per applicable laws or contracts.

Responsibilities

• Coordinate patient discharges with hospital case managers, social workers, and staff.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on equipment use and maintenance.
• Maintain accurate documentation including referrals and insurance verification.
• Support emergency equipment needs and participate in quality initiatives.
