Job summary

Rotech Healthcare Inc. is seeking a Hospital Liaison – Respiratory & DME Services to bridge hospital teams and home care. You will coordinate discharge planning with Case Management to ensure timely delivery and setup of respiratory equipment and DME at home, while educating patients and caregivers on safe operation.

In this role you will maintain documentation, support quality initiatives, and travel to patient homes using a company vehicle to ensure successful transitions to home care.

Qualifications

• High school diploma or GED required.
• Experience working in healthcare or medical equipment setting preferred.
• Strong communication and organization skills; ability to educate patients and caregivers.

Responsibilities

• Coordinate patient discharges with hospital staff and case management.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on equipment use and maintenance.
• Maintain accurate documentation including referrals, insurance verification and delivery tickets.
• Travel to patient homes using a company vehicle for setup and follow-up.
