Rotech Healthcare Inc. is seeking a Hospital Liaison to coordinate discharge planning and ensure seamless handoffs between hospital teams and home care services. You will educate patients on equipment use and support timely delivery of respiratory and DME devices.

This role requires collaborating with case management, maintaining documentation, and traveling to patient homes as needed. A healthcare background and strong communication skills are essential.

Qualifications

• High school diploma or GED required.
• Experience with medical equipment or healthcare setting preferred.
• Knowledge of medical terminology beneficial.

Responsabilités

• Coordinate patient discharges with hospital case managers and staff.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on equipment use and maintenance.
• Maintain accurate documentation including referrals, insurance verification, tickets, and logs.
• Travel to patient homes for equipment setup and follow-up using company vehicle.
