Job summary

Rotech Healthcare Inc. seeks a Hospital Liaison to bridge hospital and home care, coordinating discharge planning for Respiratory Therapy and DME services. You will work with Case Management and Continuing Care to ensure seamless transitions and educate patients and caregivers on equipment use.

The role involves coordinating deliveries, documentation, and occasional travel to patient homes, with a pay range of $19.00 to $22.00 per hour and eligible bonus opportunities.

• Experience coordinating patient discharges and home care.
• Ability to educate patients and caregivers on equipment use.

Responsibilities

• Coordinate hospital discharges with case management and care teams.
• Ensure timely delivery/setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on proper equipment use and maintenance.
• Maintain accurate documentation including referrals and delivery tickets.
• Travel to patient homes for equipment setup using company vehicle.
