Job summary

Rotech Healthcare Inc. is seeking a Hospital Liaison to bridge hospital staff and our home care services. You will coordinate discharge planning for patients requiring Respiratory Therapy and DME at home, ensuring equipment delivery and patient education are completed accurately.

The role involves working with Case Management, Continuing Care teams, and a company vehicle for home visits. A clinical coordination mindset, attention to detail, and strong communication are essential.

Qualifications

• Experience coordinating patient discharges and transitions to home care.
• Ability to educate patients and caregivers on equipment use and safety.
• Familiarity with hospital workflows and case management processes.
• Competence in documenting referrals, insurance verification, and delivery records.

Responsibilities

• Coordinate discharge planning with hospital staff and our home care teams.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients/caregivers on equipment use and maintenance.
• Document referrals, insurance verifications, and activity logs.
• Support emergency equipment needs and troubleshoot issues as they arise.
