Job summary

Rotech Healthcare Inc. is seeking a Hospital Liaison to connect hospital teams with our home care services. You will coordinate discharge planning, arrange timely equipment delivery, and educate patients and caregivers on safe equipment use.

The role involves collaboration with Case Management, Continuing Care, and field visits to patient homes. Requires high school diploma, strong English skills, and the ability to manage multiple tasks in a healthcare setting.

Responsibilities

• Coordinate patient discharges with hospital staff and case management.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on equipment use and maintenance.
• Maintain accurate documentation including referrals, insurance verification, and delivery logs.
• Travel to patient homes using company vehicle for setup and follow-up.
