Job summary

Rotech Healthcare Inc. is seeking a Hospital Liaison to coordinate patient discharges and ensure timely delivery of respiratory and DME equipment. You will work with Case Management and Continuing Care teams to support transitions from hospital to home care, educating patients and caregivers on equipment use.

The role includes travel to patient homes, shift Tue-Sat, and a compensation range of $19–$21 per hour with a bonus opportunity. Join a leader in home healthcare.

Qualifications

• Background check is required with individualized assessment.
• Drug screen where applicable for the position.
• Compliance with credentialing processes when applicable.
• Valid driver’s license in state of residence with clean record.
• High school diploma or GED required.

Responsibilities

• Coordinate patient discharges with hospital staff and home care teams.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on proper equipment use and maintenance.
• Maintain accurate documentation including referrals and delivery tickets.
• Support emergency equipment needs and troubleshoot issues as they arise.
• Participate in quality improvement initiatives to meet standards.
• Build relationships with hospital staff as a trusted discharge resource.
• Provide in-service training and CEU programs as needed.
• Travel to patient homes for equipment setup and follow-up using a company vehicle.
