Rotech Healthcare Inc. is seeking a Hospital Liaison to connect hospital staff with home care services for Respiratory & DME. You will coordinate discharges, ensure equipment delivery, and educate patients in home settings.

This role supports seamless transitions from hospital to home with a focus on safety and timeliness. The position offers a pay range of $22.50–$24 per hour with a bonus opportunity, and requires strong communication, documentation, and coordination skills to work across Case

Qualifications

• High school diploma or GED required; healthcare experience a plus.
• Experience in coordinating patient discharges and equipment delivery is preferred.
• Strong communication and documentation skills are essential.

Responsibilities

• Coordinate patient discharges between hospital teams and home care services.
• Ensure timely delivery and setup of respiratory and DME equipment at discharge.
• Educate patients and caregivers on equipment use and maintenance.
• Maintain accurate records including referrals, insurance verification, and delivery logs.
• Support emergency equipment needs and participate in quality initiatives.
