Job summary

Endeavors is seeking a Disaster Human Services Case Manager in Nashville, TN to provide survivor-centered case management during field or remote operations. Responsibilities include intake, needs assessment, recovery planning, resource coordination, and thorough documentation to support individual and family recovery while protecting privacy.

Deployable positions include housing allowances and per diems; must maintain readiness, complete required training, and adhere to program guidelines.

Qualifications

• Bachelor's degree in a social service field is preferred or equivalent experience.
• Relevant experience in case management, human services, crisis intervention, benefits/navigation, disaster recovery, or community-based services is preferred.
• Professional language capability beyond English is desirable and deployments may require language skills.

Responsibilities

• Conduct disaster survivor intake, screening, and needs assessments using approved tools and procedures.
• Develop individualized recovery plans with survivors, set goals, identify barriers, and update as needed.
• Identify, verify, and coordinate available federal, state, tribal, local, nonprofit, and community resources.
• Provide referrals and follow-up, assist survivors in navigating benefits, services, and recovery resources.
• Maintain accurate case information, progress notes, referrals, and statistics in approved systems.
• Deploy within required timeframes, field or remote work, and maintain readiness and credentialing.
• Protect PII and survivor privacy; comply with all applicable laws and security requirements.
• Demonstrate exceptional customer service, placing clients first to empower recovery.
