Job summary

Endeavors seeks a Disaster Human Services Case Manager to provide survivor-centered case management during field or remote operations. You will conduct intake, assess unmet needs, and develop individualized recovery plans in partnership with survivors while protecting privacy and promoting equitable access to services.

Responsibilities include coordinating federal, state, local, nonprofit resources, tracking progress, and maintaining complete case documentation.

Qualifications

• Bachelor's degree from an accredited college or university in Social Work, Psychology, Sociology, Human Services, Criminal Justice, Emergency Management, or a related field preferred.
• Equivalent relevant education and experience may be considered per contract position qualifications.
• Professional spoken fluency in a language in addition to English is desirable.

Responsibilities

• Conduct disaster survivor intake, screening, and needs assessments using approved tools and procedures.
• Develop individualized recovery plans with survivors and set achievable goals.
• Identify, verify and coordinate federal, state, tribal, local, nonprofit, and community resources to address unmet needs.
• Provide resource referrals and follow-up, assisting survivors in navigating benefits and resources.
• Maintain survivor records, progress notes, and required statistics in CM systems.
