Job summary

Care Lync is seeking a dedicated Support Coordinator to manage individualized care plans for adults with Intellectual and Developmental Disabilities. You will broker and coordinate services, monitor outcomes, and help clients access housing, medical, and community supports.

You will develop ISP-based plans, collaborate with other care teams, and ensure confidentiality and compliance. A valid driver’s license, CPR/First Aid, and state clearances are required for travel and on-site support.

Qualifications

BS/BA degree in related field.
Minimum of 3 years related experience providing case management for individuals with ID/Autism/Physical Disabilities.
Experience with Microsoft Office and state-maintained databases.
CPR/First Aid certification and updated state clearances.
Valid Driver's License and vehicle insurance; keep registration current.

Responsibilities

Complete assessments to identify participant needs and desires for service delivery.
Develop Person-Centered Service Plans and update annually or when changes occur.
Inform participants about provider options to enable informed choices.
Ensure ISP aligns with Behavioral and Crisis Plans where applicable.
Coordinate with other care coordinators to ensure seamless service delivery.
Travel to participants as needed (approximately 50%).
