Responsibilities

• Provides direct care and support services to individuals.
• Implements habilitation and rehabilitation, behavior management, crisis intervention, and treatment programs.
• Documents information on the person’s life and daily activities.
• Assists with medication administration and observes individuals.
• Serves as shift/team lead when the DSP IV Lead/Charge is not on duty.
• Transports individuals and accompanies staff on activities when needed.
• Maintains a safe, sanitary living environment and follows infection control procedures.
