OneWell Health Care in Levittown, PA is seeking a dedicated Caregiver/Direct Support Professional (DSP) to help individuals with IDD live independent, meaningful lives in home and community settings. This field-based role involves hands-on support, daily living activities, and transportation to appointments.

The DSP will follow individualized service plans, maintain documentation, comply with HIPAA, and participate in trainings, with flexible scheduling including evenings and weekends.

Qualificações

• High school diploma or equivalent.
• Experience in caregiving or direct support preferred.
• Ability to use electronic documentation and EVV systems.
• Proficiency with basic computer skills and mobile apps.
• English proficiency; bilingual skills are a plus.

Responsabilidades

• Provide hands‑on, person‑centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meal prep, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well‑being.
• Maintain structured routines and support transportation to appointments and community outings.
• Document accurately, participate in trainings and team meetings, and provide flexible coverage during staffing shortages.
