Job summary

OneWell Health Care in Hershey, PA is seeking a dedicated Caregiver / Direct Support Professional (DSP) to support individuals with IDD in home and community settings. This field-based, non-exempt role offers flexible scheduling.

You will implement ISP/IPP, assist with daily living activities, monitor health and safety, promote independence, and document care per HIPAA and agency standards. Collaboration with a multidisciplinary team is essential.

Qualifications

• High school diploma or equivalent.
• Prior caregiving or direct support experience preferred.
• English proficiency; bilingual a plus.

Responsibilities

• Provide hands-on, person-centered support per care plan.
• Assist with daily living activities and health monitoring.
• Support transportation to appointments and community outings.
• Maintain documentation and comply with HIPAA and policies.
• Participate in trainings and team meetings.
