The role focuses on daily living activities, health monitoring, and promoting independence and community inclusion, with flexible scheduling.

You will follow individualized service plans, document care notes, and collaborate with a multidisciplinary team.

Qualifications

• High school diploma or equivalent.
• Experience in caregiving or direct support for IDD preferred.
• Ability to use electronic documentation and EVV systems.

Responsibilities

• Provide consistent, respectful, high-quality support per care plans.
• Assist with personal care, daily living activities, meals, mobility, and medication as authorized.
• Document health, safety, and behavioral observations; maintain timely records.
• Support transportation to appointments and community activities; participate in trainings and team meetings.
