Job summary

OneWell Health Care is seeking a dedicated Caregiver/Direct Support Professional (DSP) in Monessen, PA, delivering person-centered support in home and community settings.

The DSP helps with daily living activities, health and safety monitoring, and community inclusion while promoting independence and dignity for individuals with IDD. Flexible schedule; field-based work with travel to client homes.

Qualifications

• High school diploma or equivalent is required.
• Preferred: caregiving, home health, or direct support for IDD.
• English proficiency required; bilingual a plus.
• Driver license may be required; valid license and reliable transportation.

Responsibilities

• Provide hands-on, person-centered support for individuals with IDD in home and community-based settings.
• Assist with personal care, daily living activities, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
• Maintain accurate and timely documentation; participate in trainings and team meetings; support transportation to appointments and community events.
