Job summary

OneWell Health Care seeks a compassionate Caregiver/Direct Support Professional (DSP) in Thorndale, PA. This field-based role provides direct care to individuals with IDD in home and community settings, with a flexible schedule and non-exempt status under PA law.

The DSP will assist with daily living activities, health monitoring, and community participation while ensuring dignity and independence. Strong documentation, HIPAA compliance, and ability to follow ISP/IPP plans are essential.

Qualifications

• High school diploma or equivalent.
• English proficiency required (spoken and written).
• Experience in caregiving or home health preferred.

Responsibilities

• Provide hands‑on, person‑centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, personal care, meal support, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well‑being.
• Maintain documentation, participate in trainings, and support transportation as needed.
