Job summary

OneWell Health Care in Monroeville, PA seeks a dedicated Caregiver / Direct Support Professional (DSP) to support individuals with IDD in home and community settings.

The DSP will assist with daily living activities, monitor health and safety, implement ISP/IPP plans, and promote independence. This role requires reliability, strong documentation, and flexibility to work evenings or weekends as needed.

Qualifications

• High school diploma or equivalent is required.
• Experience in caregiving or direct support for IDD preferred.
• Ability to use electronic documentation and EVV systems.

Responsibilities

• Provide hands-on, person-centered support according to care plans.
• Assist with daily living activities, personal care, meal prep, mobility, and medication as authorized.
• Support transportation to appointments and community activities.
• Document observations and activities accurately and timely.
• Participate in trainings and team meetings.
