Job summary

OneWell Health Care is seeking a compassionate Caregiver / Direct Support Professional (DSP) to support individuals with IDD in home and community settings in Sarver, PA. This field-based, flexible schedule role focuses on direct care, skill development, and promoting independence.

You will work under supervision to implement ISP/IPP plans, assist with daily living, and monitor health and safety while maintaining confidentiality and precise documentation.

Qualifications

• High school diploma or equivalent required.
• English proficiency in speaking and writing is required; bilingual skills are a plus.
• Experience in caregiving, home health, or direct support is preferred.

Responsibilities

• Assist with personal care and daily living activities.
• Support meal preparation, nutrition, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health and safety.
• Promote independence, dignity, and community participation.
• Maintain accurate documentation and participate in trainings and team meetings.
