Job summary

OneWell Health Care in Pennsylvania is seeking a dedicated Direct Support Professional (DSP) / Caregiver for field-based assignments in home and community settings. You will provide hands-on support with daily living activities, meal preparation, mobility, and health monitoring to individuals with IDD.

The role emphasizes dignity, independence, and inclusion, requires documentation, adherence to ISP/IPP plans and HIPAA, and offers flexible scheduling with opportunities for training and career

Qualifications

High school diploma or equivalent.
Experience in caregiving, home health, or direct support is preferred.

Responsibilities

Provide hands-on, person-centered support in home and community-based settings.
Assist with personal care, daily living activities, meals, mobility, and medications as authorized.
Implement behavior support strategies and monitor health, safety, and emotional well-being.
Promote independence, routines, and community participation including employment or vocational activities.
Maintain accurate and timely documentation and participate in trainings and team meetings.
