Job summary

OneWell Health Care is seeking a dedicated Direct Support Professional (DSP) in Lewistown, PA to provide person-centered, in-home and community-based support for individuals with IDD. The DSP will implement ISP plans, support daily living activities, and promote independence with dignity and safety, under supervision.

Qualifications include a high school diploma or equivalent, English proficiency, and experience in caregiving preferred.

Qualifications

• High school diploma or equivalent.
• Prior caregiving experience preferred (IDD/HH/DS) in community-based or residential settings.
• Experience with home health or direct support for IDD is a plus.

Responsibilities

• Provide consistent, respectful, high‑quality support in accordance with each individual’s care plan.
• Assist with personal care and daily living activities.
• Support meal preparation, mobility, and medication assistance as authorized; implement behavior support strategies.
• Maintain accurate and timely documentation; participate in trainings and team meetings.
