Job summary

OneWell Health Care is seeking a dedicated Caregiver/Direct Support Professional (DSP) in Spring Creek, PA to support individuals with IDD in home and community settings. This field-based, flexible-position focuses on daily living activities, skill development, health and safety monitoring, and community participation with emphasis on dignity and independence.

Qualifications

• High school diploma or equivalent.
• Preferred: experience in caregiving, home health, or direct support for individuals with IDD.
• Experience in community-based or residential support settings is preferred.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meal prep, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and wellbeing.
• Maintain structured routines and support employment or vocational activities.
• Document all care activities accurately and timely.
