Job summary

Bridgewell is seeking a Direct Support Professional on a per diem schedule in Peabody, MA. You will provide hands-on assistance to individuals with developmental, physical, and psychiatric disabilities in residential settings, focusing on daily living skills and safety.

You will document progress, coordinate with clinicians and guardians, and help participants access community resources. A valid US driver’s license and reliable transportation are required.

Qualifications

• High school diploma or GED required or 5+ years of verified experience.
• Experience with developmental or psychiatric disabilities preferred.
• Ability to drive and transport clients in wheelchair vans.

Responsibilities

• Provide daily living skills support and supervision in residential settings.
• Encourage independence and decision-making in service users.
• Maintain documentation per DDS, DMH, MassHealth, HUD, DPH, CARF standards.
• Assist with medical, clinical, and financial needs.
• Transport and accompany clients to activities and appointments.
• Attend staff meetings and comply with Bridgewell policies.
• Maintain training certifications (FA/CPR, PABC, MAP).
