Qualifications

• High school diploma or equivalent is required.
• Experience as a Direct Support Professional, caregiver, CNA, HHA, PCA, or PCT preferred.
• English proficiency required; bilingual skills preferred.

Responsibilities

• Provide direct care in home and community settings per ISP/IPP.
• Assist with daily living activities, mobility, meals, and safety.
• Document care using EVV and other required records.
• Follow infection control, safety procedures, and care plans.
• Coordinate with care team and report changes.
