Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to empower individuals with IDD in home and community settings. The DSP delivers hands-on, person-centered care, supporting daily living activities, health and safety monitoring, and community participation while respecting dignity and independence.

The role operates in field-based environments with flexible scheduling, requiring adherence to ISP/IPP plans and HIPAA/privacy standards.

Qualifications

• High school diploma or equivalent required.
• Experience in caregiving, home health, or direct support preferred.
• Ability to travel to individual homes and community sites as needed.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home/community settings.
• Assist with activities of daily living, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and well-being.
• Maintain accurate documentation and participate in trainings and team meetings.
