Job summary

Responsibilities

• Provide direct, person-centered care in home and community settings.
• Assist with personal care, daily living activities, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
• Maintain structured routines, document accurately, and participate in trainings and team meetings.
