Job summary

Dialyze Direct is seeking a Registered Nurse for full-time day shifts. The role focuses on providing quality dialysis nursing, training, and home support as needed. Collaboration with nephrologists, SNF reps, and Dialyze Direct team is essential.

You will collect patient data around hemodialysis, maintain care plans, document in EMR, and handle related clerical tasks. The ideal candidate holds a valid RN license, with 6 months RN experience and 3 months dialysis exposure preferred; BLS

Qualifications

• 6 months RN experience preferred.
• 3 months hemodialysis experience preferred.
• BLS certification required.

Responsibilities

• Provide nursing care, training, and home support as needed.
• Collaborate with Dialyze Direct team, SNF representatives, and nephrologists.
• Collect and record patient health data before/after hemodialysis.
• Develop and maintain a care plan to meet outcomes.
• Document interventions in EMR and manage labs.
• Perform clerical tasks and manage inventory.
• Administer medications and ensure safety.

Schedule:

• Full Time
• Day shift only
• Fixed schedule/no on call

Key Responsibilities:

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation.

Requirements:

• Valid RN license
• 6 months of RN experience strongly preferred
• 3 months of hemodialysis experience
• BLS certification (American Red Cross or American Heart Association Required); and
• Ability to lift up to 35 pounds is required
• This is a safety sensitive position
