Job summary

Dialyze Direct is seeking a Registered Nurse to provide dialysis-related nursing care and support. The role requires 12 months of direct dialysis experience within the last two years, a valid RN license, and BLS certification.

The nurse will function with minimal oversight, collaborate with multidisciplinary teams, and handle EMR documentation. The position includes day shift, Monday–Friday, full-time, with occasional travel to nearby locations.

Qualifications

• RN with 12 months dialysis experience in the last 24 months.
• Valid RN license and BLS certification.
• Ability to lift up to 35 pounds and perform patient care tasks.
• This is a safety sensitive position.

Responsibilities

• Provide nursing care, training and home support as needed with minimal supervision.
• Collaborate with Dialyze Direct team, skilled nursing home facilities and nephrologists.
• Record patient health data during pre, intra, and post-dialysis.
• Develop and maintain a care plan to reach outcomes.
• Perform lab collection and processing and reschedule missed collections.
• Manage clerical tasks and patient charting; maintain a positive attitude.

Schedule :

• Full Time- Monday through Friday
• Day shift only

Key Responsibilities:

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory; management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation

Requirements:

• Each RN shall have, at minimum, 12 months of experience in the last 24 months in the direct provision of dialysis
• Valid RN license
• BLS certification; and
• Ability to lift up to 35 pounds is required
• This is a safety sensitive position
