Job summary

Dialyze Direct is seeking a registered nurse for full-time, day shift in Willow Grove, PA. The role involves providing high-quality dialysis care, patient assessment, and home support as needed, with travel to nearby locations up to 50%.

Requirements include an active RN license, BLS certification, and 6 months of RN experience preferred; hemodialysis experience preferred. You will work with a multidisciplinary team to deliver compassionate care and maintain excellent patient outcomes.

Qualifications

• Valid driver's license and transportation.

Responsibilities

• Must be able to function with little oversight to provide nursing care, training, and home support (as needed).
• Collaborates with Dialyze Direct team members, facility representatives and nephrologists.
• Collects and records patient health data during pre/post hemodialysis.
• Develops and maintains a plan of care to achieve outcomes.
• Implements and records interventions in EMR to attain outcomes.
• Performs lab collection, processing and shipping; reschedules missed collections.
• Handles clerical tasks in addition to patient care.
• Maintains a positive, respectful attitude toward patients and colleagues.
• Occasional travel to nearby locations.
• Medication administration and documentation.

Schedule:

• Monday-Friday
• Full-Time
• Day shift only

Key Responsibilities:

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation.

Requirements:

• Valid RN license
• 6 months of RN experience strongly preferred
• 3 months of hemodialysis experience strongly preferred
• Valid drivers license and transportation
• Ability to travel covering multiple locations
• BLS certification (American Red Cross or American Heart Association Required); and
• Ability to lift up to 35 pounds is required
• This is a safety sensitive position
