Job summary

Dialyze Direct is seeking a registered nurse to provide high-quality dialysis care in Chicago, IL. This full-time, day-shift role requires an active RN license, BLS, and dialysis experience; travel up to 75% to nearby facilities is expected.

You will work with nephrologists and the Dialyze Direct team, perform patient assessments, document in EMR, collect labs, and support home dialysis as needed. Compensation is based on education and experience, with benefits including 401(k), FSA, education

Qualifications

• Active RN license in Illinois and BLS certification.
• 6 months RN experience preferred.
• 3 months hemodialysis experience.
• Ability to travel up to 75% to nearby facilities.

Responsibilities

• Provide dialysis nursing care with minimal supervision.
• Collaborate with dialysis team, SNF reps, and nephrologists.
• Record patient data before, during, and after treatment in EMR.
• Develop and maintain care plans to meet outcomes.
• Perform lab collection and handling; manage clerical tasks.
• Maintain positive attitude with patients and colleagues.
• Occasional travel to nearby locations.
• Administer medications and document.

Schedule:

• Full Time
• Day shift only

Key Responsibilities:

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation.

Requirements:

• Valid RN license
• 6 months of RN experience strongly preferred
• 3 months of hemodialysis experience
• In the state of Texas, 6 months of hemodialysis experience within the last 24 months
• BLS certification (American Red Cross or American Heart Association Required); and
• Ability to lift up to 35 pounds is required
• This is a safety sensitive position
