Job summary

Dialyze Direct is seeking a Registered Nurse to deliver high-quality dialysis care and patient support. The role emphasizes independence, critical thinking, and collaboration with a multidisciplinary team in a fast-paced environment.

The position is full-time with a day shift and substantial travel to nearby locations. A valid RN license, BLS, and dialysis experience (3–6 months preferred) are required, with lifting ability up to 35 pounds.

Qualifications

• 3 months of hemodialysis experience.
• BLS certification (American Red Cross or American Heart Association Required).

Responsibilities

• Provide nursing care, training, and home support as needed.
• Collaborate with Dialyze Direct team members, facility representatives, and nephrologists.
• Record patient health data during pre/post hemodialysis.
• Develop and maintain an individualized plan of care.
• Document interventions in the EMR.
• Collect, process, and ship laboratory specimens as required.
• Perform clerical tasks and maintain patient charts.
• Maintain a positive attitude toward patients and colleagues.
• Occasional travel to nearby locations.
• Administer medications and document administration.

Schedule:

• Full Time
• Day shift only

Key Responsibilities:

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation.

Requirements:

• Valid RN license
• 6 months of RN experience strongly preferred
• 3 months of hemodialysis experience
• BLS certification (American Red Cross or American Heart Association Required); and
• Ability to lift up to 35 pounds is required
