Job summary

Dialyze Direct is seeking a licensed Registered Nurse for a part-time, day-shift role in Virginia. The position requires travel to nearby facilities (approximately 50%), with responsibilities including direct patient care during hemodialysis, medication administration and EMR documentation.

The ideal candidate will have 6 months of RN experience, 3 months of hemodialysis experience, BLS certification, a valid driver’s license, and the ability to lift up to 35 pounds.

Qualifications

• Valid RN license is required.
• 6 months of RN experience strongly preferred.
• 3 months of hemodialysis experience.
• Valid driver’s license and transportation.
• Ability to travel to multiple locations.
• BLS certification (American Red Cross or American Heart Association).
• Ability to lift up to 35 pounds; safety sensitive position.

Responsibilities

• Provide nursing care, training and home support with minimal oversight.
• Collaborate with Dialyze Direct team, SNF facility reps and nephrologists.
• Collect and record patient health data during/after hemodialysis.
• Develop and maintain a care plan to attain outcomes.
• Implement and record interventions in EMR.
• Perform lab collection, processing, and shipping; reschedule missed collections.
• Perform clerical tasks in addition to patient care.
• Maintain a positive attitude with patients and colleagues.
• Occasional travel to nearby locations.

Schedule

• Part Time, Tuesday & Thursday
• Day shift only

Key Responsibilities

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation.

• BLS certification (American Red Cross or American Heart Association Required); and
• Ability to lift up to 35 pounds is required
