Job summary

Dialyze Direct is seeking a Registered Nurse for full-time work in Virginia with a two-year degree requirement and a 75% travel expectation. The role focuses on delivering dialysis care, data collection, and home support with a fixed schedule and day shifts in Charlottesville and Lynchburg.

You will collaborate with nephrologists, nursing home facility staff, and Dialyze Direct teams, documenting care in EMR and performing lab duties as needed. A valid RN license and BLS are required.

Qualifications

• Valid RN license.
• 6 months of RN experience preferred.
• 3 months of hemodialysis experience.
• BLS certification required.
• Ability to lift up to 35 pounds.
• This is a safety sensitive position.

Responsibilities

• Must provide nursing care, training, and home support with minimal oversight.
• Collaborate with Dialyze Direct team, SNFs, and nephrologists.
• Collect and record patient health data before/after hemodialysis.
• Develop and maintain a care plan to reach expected outcomes.
• Implement and record interventions in EMR.
• Perform lab collection, processing, and shipping; reschedule labs.
• Handle clerical tasks (filing, inventory, admissions, patient charts).
• Maintain a positive and respectful attitude toward patients and colleagues.
• Occasional travel to nearby locations; administer medications.

Schedule:

• Working two days a week in Charlottesville and two days a week in Lynchburg.
• Full Time
• Day shift only
• Fixed schedule/no on call

Key Responsibilities:

• Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
• Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
• Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
• Develops and maintains a plan of care that describes interventions to attain expected outcomes;
• Implements and records interventions to attain expected outcomes in EMR;
• Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
• Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
• Maintains a positive and respectful attitude to all patients and colleagues;
• Occasional travel to other nearby locations; and
• Medication administration and documentation.

Requirements:

• Valid RN license
• 6 months of RN experience strongly preferred
• 3 months of hemodialysis experience
• BLS certification (American Red Cross or American Heart Association Required); and
• Ability to lift up to 35 pounds is required
• This is a safety sensitive position
