Exciting opportunity to join a team of highly motivated and energized professionals. Must be able to function at a Full-Time basis. We are open Monday-Wednesday-Friday schedule, provide nursing care, training, and home support (as needed), work collaboratively with team members, and skilled nursing home facility representatives; collect and record relevant patient health data (EMR), and develop patient care plans.
