Qualifications

• Experience with individuals having mental health diagnoses, developmental disabilities, or substance use disorders depending on population served.

Responsibilities

• Assess individuals for their hopes and goals and provide supports to help achieve them.
• Assist in developing relationships in the community and with natural supports.
• Support participation in community activities aligned with the person's interests.
• Protect and promote the rights of individuals; assist with advocacy and planning processes.
• Provide emotional, physical, and medical support as needed.
• Maintain person-centered, positive relationships and supportive approaches.
