Qualifications

• High School Diploma required.
• Valid driver's license in the USA.
• Experience with individuals who have mental health diagnoses or developmental disabilities is preferred.

Responsibilities

• Assess individuals’ hopes and goals and provide supports to achieve them.
• Help individuals build relationships in their community and with natural supports.
• Assist with community participation aligned to the person’s interests.
• Protect and promote rights; support advocacy and personal advocates.
• Complete documentation, including progress notes and incident reports.
• Drive and provide transportation as required; sleepovers may be needed.
