Job summary

Responsibilities

• Assess each person’s hopes, dreams, and desires and provide supports to help achieve them.
• Help individuals develop relationships in their community and with natural supports.
• Assist individuals to participate in community life aligned with their interests.
• Protect and promote the rights of individuals and support advocacy efforts.
• Provide emotional, physical, and medical support as needed.
• Document progress and complete required logs and progress notes.
