Job summary

Brookdale Senior Living in Tempe is seeking a Clare Bridge Program Assistant to support dementia-centered programming and resident engagement. The role involves implementing person-centered activities, assisting in daily care tasks, and collaborating with the Program Manager to deliver meaningful experiences for residents.

The position emphasizes teamwork, adherence to Brookdale standards, and involvement in community events.

Qualifications

• High School Diploma or equivalent required.
• Associates Degree preferred.
• Minimum one year of experience with older adults and dementia preferred; or equivalent combination of education and experience.
• Must have a valid driver’s license and a clean 3-year driving abstract.
• Physically able to stand, walk, and lift up to 50 pounds.
• Basic knowledge of Windows and word processing software.

Responsibilities

• Assists Clare Bridge Program Manager or Coordinator in leading person-centered dementia programs.
• Engages residents in seven well-being domains within a dementia-friendly environment.
• Reviews resident service plans and completes program documentation.
• Ensures programs comply with state, federal, and Brookdale standards.
• Coordinates family events, open houses, and community outreach activities.
• Provides guidance to associates on programming and residents’ quality of life.
• May assist with travel and driving for community events and volunteer activities.
