Job summary

ADAPT Community Network is seeking a Program Coordinator to oversee daily Day Services operations. You will lead a multidisciplinary team, ensure person‑centered services, and implement individualized plans with strict adherence to regulatory standards.

Qualifications

• Bachelor’s degree in Human Services or related field.
• Minimum of one year post-graduate experience; three years preferred.
• At least two years of supervisory experience, preferably in developmental disabilities.
• Driver’s license preferred (New York).
• Strong understanding of OPWDD regulations and person-centered planning.
• Excellent communication, interpersonal, and organizational skills.

Responsibilities

• Provide day-to-day supervision of program activities and ensure services align with interdisciplinary plan.
• Coach, supervise, evaluate staff, and manage performance issues as needed.
• Oversee development and implementation of individualized treatment plans and curricula.
• Ensure accurate documentation of treatment plans and progress notes in compliance with regulations.
• Develop and implement program curricula and structured activities to support daily living, social, and vocational skills.
• Facilitate staff trainings and mentor team members to promote best practices.
